Families can complete the Ages and Stages Questionnaire (ASQ) to reflect their observations of their child's learning and growth. Please note that respondent selection is managed by the school or program through their Questionnaire Settings. Families will have access to the questionnaire only when the school or program selects them as a respondent.

Accessing the Family Questionnaire
Depending on a family member’s Communication Settings, the Family Questionnaire can be accessed and completed through the Kaymbu for Families app or through email (web version).
- Please note that families can also customize their notification preferences directly from the family app by navigating to their profile tab. If you do not have access to the Family Questionnaire, we recommend contacting your child’s teacher or a school director for additional information.
Email (Web Version)
To access the web version of the Family Questionnaire, follow the steps below:
- Open the email sent by your child's school/program with the subject title “Ages & Stages Questionnaire”
- Select “Complete the Questionnaire”; this will lead you to the web version of the Family Questionnaire
Kaymbu for Families App
To access the Family Questionnaire on the Kaymbu for Families app, follow the steps below:
- Tap on the ASQ tab
- Note: From the ASQ tab, families can view the progress status of their questionnaire (Not Started, In Progress, Submitted, or Completed) and they can learn more about ASQ by tapping on the link above at the top of their screen.
- Select your child’s ASQ profile to open up the questionnaire
Answering the Family Questionnaire
Before starting the questionnaire, take the time to observe your child in their day-to-day activities and interactions. To ensure the most accurate results, it is important to answer each question as honestly as possible based on what you regularly observe. These observations provide valuable insights that help create a more complete picture of your child’s development.
The questionnaire will include questions about your child’s development covering five main categories: Communication, Gross Motor, Fine Motor, Problem-Solving, and Personal Social skills.
Terms of Agreement
When accessing the questionnaire for the first time, families will be required to complete the Terms of Agreement acknowledging that they have read the provided information regarding the Ages and Stages Questionnaire. Once completed, families can officially begin answering the questionnaire.
Required Fields
Throughout the questionnaire, required fields are indicated with a red asterisk. Required fields as well as all questions within each developmental category must be filled out before you can proceed to the next question.
Navigating the Family Questionnaire
To move on to the next question, simply select “Next” at the bottom of the page, or select “Previous” (or the back arrow) to go back to an earlier question.
As families complete the questionnaire, the system will automatically save their progress. This means you can navigate away from the questionnaire at any time, knowing that the information you have entered will remain.
Please note that selecting “I Don’t Know” as a response is allowed three times per developmental category. If “I Don’t Know” has been submitted more than three times, the category will be marked as “Not Scored”.
Users can exit the questionnaire at any time by selecting the “X” icon in the top-right corner of the screen.
Review and Submit Responses
At the end of the questionnaire, families will be able to review all of their responses before officially submitting them. To make any changes, simply click the pencil icon next to the relevant response.
Once all responses have been reviewed, select the “Confirm Answers” button to officially submit your responses to your child’s teacher.
Please note that once the Family Questionnaire is submitted, no further changes can be made.
ASQ Family Report
Once the Family Questionnaire has been completed and submitted, your child's teacher will review all responses, and then the ASQ Family Report will be shared with you.
The ASQ Family Report compiles responses from the Teacher Questionnaire and/or Family Questionnaire into a student-specific report. This report will include scores for each developmental category (Communication, Gross Motor, Fine Motor, Problem Solving, and Personal-Social), as well as recommended activities to promote your child's continued growth and development.
