A child's ASQ profile can be accessed through the ASQ Dashboard, where individual data can be managed and reviewed. By accessing a child's ASQ profile, users can send reminders to the family, review Family Questionnaire responses, complete the Teacher Questionnaire, and compare results.

Accessing ASQ Student Profile
From the ASQ Dashboard, users can click on a student’s name to view their profile and access individual data.
Viewing ASQ Student Profile
When viewing a student’s ASQ profile, teachers can access the Teacher Questionnaire and monitor the progress status of the Family Questionnaire for that particular student.
- Please note that respondent selection is managed through a screening group’s Questionnaire Settings. Depending on the screening group's questionnaire settings, respondents can be set to teachers, families, or both.
Family Questionnaire
The Family Questionnaire section on a child's ASQ profile is where users can track the Family Questionnaire progress for that student, and will be labeled as follows: Not Started, In Progress, Submitted, or Completed.
- Once the Family Questionnaire has been submitted, teachers will be able to view their responses.
If the Family Questionnaire due date is approaching, teachers can choose to manually send a reminder to a particular family, rather than the entire screening group.
To send a reminder to an individual family, follow the steps below:
- Navigate to Menu > ASQ
- High-level users, such as directors or administrators, will need to dial into the specific organization > center > classroom.
- At the classroom level, click on the student’s profile on the ASQ Dashboard
- In the Family Questionnaire section, click on “Actions”
- Select “ Send Reminder”
Teacher Questionnaire
The Teacher Questionnaire section on a child’s ASQ profile is where teachers can access and answer the Teacher Questionnaire and monitor its status. The progress status will be labeled as follows: Not Started, In Progress, Submitted, or Completed.

Notes
The Notes section of a student’s ASQ profile allows teachers to manually document additional observations, reminders, or relevant information. Notes entered in this section are for internal reference only and will not appear on the ASQ Family Report.
Reviewing and Comparing Results
When a family member or teacher submits their responses, the results can be reviewed by accessing a student’s ASQ profile.
- To view submitted responses, select "View" within the corresponding section of the child's ASQ profile.
ASQ Skills Overview
The ASQ Skills Overview section of a student’s profile provides a comparison of scores across the five developmental domains. This data is populated once the questionnaire has been submitted by the teacher or family member.
The scores are derived from the questionnaire responses and are compared with developmental benchmarks, and help to identify strengths and areas where additional support may be needed.
Follow-up Actions Taken
Following the submission of the questionnaire, teachers can document any follow-up steps taken to support the child’s development under the Follow-up Actions Taken section of the child's ASQ profile.
- This includes tracking rescreening activities, sharing results with the child’s health care provider, community agency, or early intervention program, and recording referrals related to health concerns such as hearing, vision, or behavioral needs.
Keeping this information up to date helps ensure that important support efforts are documented and accessible for future reference. Please note that the information entered in this section is for internal reference and will not appear on the ASQ Family Report.
Marking the Questionnaire as Complete
Once responses have been officially submitted for the Teacher Questionnaire and/or Family Questionnaire, the option to mark the questionnaire as complete will become available on a student’s ASQ profile.
To mark a questionnaire as complete, follow the steps below:
- Navigate to Menu > ASQ
- High-level users, such as directors or administrators, will need to dial into the specific organization > center > classroom.
- At the classroom level, click on the student’s profile on the ASQ Dashboard
- Select “Mark Complete” at the bottom of the student’s profile
Please note that if a family has not completed the Family Questionnaire and a teacher marks it as complete, the family will no longer be able to submit their responses.
When a child's profile has been marked as complete, the student’s ASQ Family Report will be automatically generated and can be accessed from the student’s ASQ profile.

Marking the Questionnaire as Incomplete
If a questionnaire has been marked as complete but changes to responses need to be made for whatever reason, it can be reopened by marking it as incomplete.
To mark a questionnaire as incomplete, follow the steps below:
- Navigate to Menu > ASQ
- High-level users, such as directors or administrators, will need to dial into the specific organization > center > classroom.
- At the classroom level, click on the student’s profile on the ASQ Dashboard
- Click on the three dots located to the right of the student’s profile avatar and select “Mark Incomplete”
Please note that marking a questionnaire as incomplete will reopen it, allowing responses to the Teacher Questionnaire and/or Family Questionnaire to be updated.
